Please visit the Practice to collect a New Patient Registration Form.

If you would prefer to have a registration form emailed to you to print and complete at home, please contact the surgery and we will be happy to email a copy to you. 

When you collect a registration form or contact us to request a form, we will make an appointment for you to come in to the Practice to meet us and be registered as a patient.

When you come in to meet us, we will go through everything you need to know about Charnwood GP Practice, answer your questions and find out how we can help you. 

When you come in to the surgery to register, please bring a Photo ID and a proof of address. 

If you are registering a child under 16, please also bring in your child's birth certificate and immunisation record.
